You will not be required to appear in court if you complete this form and e-mail it to the court at least five business days before your scheduled court date. Your in-person hearing date/time will still appear on the calendar.

If your statement is submitted in an untimely manner, you could be listed as a failure to appear, which could result in additional assessments.

The court will review your statement and the police officer's sworn statement to make a decision. By submitting a written response by either mail or e-mail, you waive your right to appeal. Moreover, by submitting a written response by e-mail, you agree to receive the court's decision via e-mail.
